After you leave hospital

There are services you can access to get well and stay well.

When you are ready to leave hospital, you will be given discharge paperwork.This is really important because it has information that will help you understand what your next steps are for care. We want you to recover as much as you can and stay well while you are in the community.

Information on your discharge paperwork may include follow-up appointments with a GP or community therapy programs. We really encourage you to attend these appointments so you can stay well at home with your family and friends.

However, if you start to feel unwell again, don’t hesitate to see a doctor. They’ll make sure you are able to connect with the best services or advise you if you have to come back to LRH for further care.
