Digital Cardiac Rehab (available until 28 April 2023)

Cardihab’s digital cardiac rehab is delivered via the SmartCR smartphone app which can be downloaded from the Apple App Store or Google Play.
A patient has the flexibility to complete the program at a place and a time that suits them. The will be supported by a Registered Nurse at Latrobe Regional Health with a weekly one-to-one phone call.
Cardiac Rehab has been demonstrated to improve quality of life and may reduce the chances of experiencing a further cardiac event. It is strongly recommended all patients who have a cardiac event participate in a 6-8 week program of cardiac rehabilitation soon after they are discharged.
Using the Digital Cardiac Rehab SmartCR app will help the patient

  • complete cardiac rehabilitation with fewer visits to the hospital
  • exercise at home
  • record and keep track of their medications
  • learn about heart health and recovery with videos and online articles.
  • manage stress and access guided relaxation audio
